United South and Eastern Tribes, Inc. (USET) is a nonprofit, inter-tribal organization that collectively represents Tribes at the regional and national level. USET consists of 25 federally recognized Tribes, and facilitates various workgroups and committees by providing a forum for the exchange of ideas and information among Tribes, agencies and governments. USET is dedicated to enhancing the development of Indian Tribes by improving the capabilities of Tribal governments and assisting member Tribes with public policy issues that influence the needs of Indian people.

Abstract

American Indian and Alaska Native (AI/AN) populations disproportionately suffer from chronic illnesses such as obesity, heart disease and diabetes. The Nashville area experiences an abundant burden of these chronic diseases. More than 50 percent of AI/AN children in the Nashville area are either overweight or obese, and the overall age-adjusted diabetes prevalence rate is 23 percent. Socioeconomic conditions such as age, educational attainment, and income level compound these issues, highlighting the great need for public health interventions in the Nashville area.

The United South and Eastern Tribes, Inc. (USET) project strives to strategically address AI/AN health disparities that plague Indian country by increasing its organizational capacity to monitor health status and improving public health infrastructure within this community. USET will achieve these long-term outcomes by implementing strategies that pertain to the following overarching objectives: (1) developing an interactive PHD-Portal to serve as a data monitoring center, a resource for stakeholders, and a channel for collaborative communication among users; (2) creating a Tribal PHD-Portal Advisory Council (TPAC), a community partner network, which will define and monitor the reduction in health disparities; and (3) increasing interest among Tribal students in health careers by introducing them to the PHD-Portal and involving them with project efforts.

The initial target population for this project consists of approximately 75,000 AI/ANs residing within the Nashville Area Indian Health Service (IHS) region and eligible for care through the Indian Health Care Delivery System. However, once the infrastructure and foundation of the portal is in place, it may be scaled to serve all AI/AN populations on a national level. The main project implementation strategies include creating the PHD-Portal, forming TPACs, and coordinating with Tribal education officials to recruit Tribal students to assist with program implementation. 

The grantee will utilize both formative and process evaluation measures to assess progress towards meeting program objectives. Specific data sources include key informant interviews, electronic tracking of data use, end-user and student surveys, and databases that track health improvement initiatives, as well as dissemination of products. For example, the number of successful installations of the PHD-Portal will be used to assess the implementation of the Data Portal integrated system (objective 1); the number of members recruited and meetings held biannually will be utilized to evaluate the TPAC partnership development (objective 2); and the percentage of youth who expressed interest and intentions to pursue public health, will help determine progress toward creating a public health career pathway for AI/ANs (objective 3). Anticipated program outcomes and impacts include:  enhanced knowledge of health disparities; increased importance of data for decision making; improved communications and sharing of information between tribes; and increased representation of AI/ANs in public health fields.
