Abstract

This State Partnership project seeks to address significant disparities in heart disease, stroke and obesity within minority populations, particularly African Americans and Latinos. In Texas, these health disparities are mainly a result of disparities in access to health care and adverse social determinants of health. To address these issues, this project will link existing statewide networks and network partners to policy-change strategies, and build the capacity of these networks to move effectively into the policy arena with policy actions relevant to community needs. The project will also provide technical support to enhance the leadership skills and capacities of communities in eliminating health disparities. The activities include community engagement, development of learning modules, collaborative development of policies and initiatives and evaluation methods that fully involve community participants in identifying program success. The intended outcomes include the creation of action plans, participation of network members in state-level policy discussions and the development of community-driven policy initiatives. In order to determine if such outcomes are reached, the project will conduct an evaluation that employs scientifically-rigorous methods consistent with best practices in the measurements of efficacy and degree of goal accomplishment. Surveys and community-based assessments will be used to collect data.
