Abstract

Oregon's Promoting Health Equity Program addresses the need for communities to have the ability to monitor the health of their constituents and develop and implement effective programs targeting health risks specific to the communities. The focus of this program will be on African-American, Asian/Pacific Islander, Latino, Native American and Alaskan Native populations who bear a disproportionate disease burden. To this end, this project will partner with Northwest Health Foundation, the Archimedes Movement and the Conference of Local Health Officers to build the capacity of local groups to collaborate, gather data and determine local priorities. They also will plan strategic initiatives, activities and policies to create lasting improvements for the health and well-being of the communities. These Equity Coalitions will consist of individuals and organizations across multiple jurisdictions and sectors in order to gain an overall, holistic perspective of the community. The intended outcomes of this project include regional agreements on local health priorities, best practices that are created and shared, in order to address social determinants of health and equity, effective collaboration, and increased knowledge of social determinants of health and equity. In order to determine if such outcomes are reached, data will be collected on a range of performance measures, including the number of Regional Equity Coalitions established, the level of collaborative engagement and ownership of individual entities participating in coalitions, and the number of strategic/work plans created that specifically address social determinants of health and equity. Pre/post assessment will be conducted to measure the effects of collaborative activities and community/regional health status. Reports capturing the administrative and organizational aspects of Coalition efforts will also be produced.

• Strengthened leadership and coordination to leverage resources and enhance effectiveness and efficiency of individual and collective efforts (including, but not limited to, research and data)
