Abstract

The State Partnership project seeks to improve health care access for rural minorities by providing adequate health insurance coverage, promoting service utilization, and addressing uneven service quality. The project intends to increase health insurance among rural minorities, particularly African-Americans, American Indians, Asian-Americans/Pacific Islanders and Hispanics. Specific activities include strengthening individual knowledge and skills that lead to healthy behaviors and promoting community education through workshops, training, webcasts, health fairs, health screenings and nutrition/health information dissemination. The activities will also educate providers to address health disparities and ensure delivery of culturally competent care, foster coalitions and networks, and change organizational practices. The intended outcomes include increasing information on the health care needs of the targeted rural minority populations, increasing public insurance program enrollment, increasing health service utilization, improving health behaviors, increasing quality of care, and improving provider/patient interactions. The project will utilize needs assessments, case studies and site visits to collect the number of eligible rural minorities enrolled in pubic insurance programs, the number of referrals and the number of rural minorities participating in learning events.
