The Virginia Department of Health Office of Minority Health and Health Equity (OMHHE) addresses health priorities and health concerns of minority populations in Virginia. The mission of the OMHHE is to identify health inequities, assess their root causes and address them by promoting social justice, influencing policy, establishing partnerships, providing resources and educating the public.

This State Partnership project seeks to build evidence for effective public health practices and policies designed to address health disparities in Virginia. Health disparities exist among minority populations, particularly African Americans, Hispanics and Asians. This project will improve planning, coordination, collaboration and linkages among public and private entities that address minority health disparities. More specifically, the project will monitor health status to identify community health problems, diagnose and investigate health problems in the community, inform people about health issues, mobilize partnerships to solve health problems and develop policies and plans that support community health efforts. The intended outcomes include increasing collaboration and technical assistance for state agencies and minority communities and, ultimately, reducing health disparities. The project will measure the effectiveness of the project by collecting such data as the number of partnerships facilitated or established, the amount of resources leveraged through partnerships to promote health equity and the number of strategies or interventions developed for addressing community and systems-level factors.
