The Wisconsin State Partnership Program will improve the health of minority and tribal communities by addressing the socio-economic determinants affecting poor health outcomes in minority communities throughout Wisconsin. The program will focus on HIV/AIDS and infant health, which are particular burdens among Wisconsin's minority populations. To this end, the Wisconsin Department of Health Services (WDHS) will establish a health disparities network to promote partnerships between bureaus and programs within WDHS on issues related to health disparities. WDHS will also invent and track existing public health initiatives focused on reducing health disparities, hold two annual community forums to promote community engagement, and develop a standardized data collection system on racial and ethnic disparities. Finally, WDHS will administer four mini-grants to implement evidence-based strategies to reduce disparities in HIV/AIDS and infant mortality. In order to measure program progress, partners will be surveyed and records will be kept of information, including the number of participants and number of invitee letters. Additionally, community engagement will be gauged by a subcommittee formed by WDHS.
