The Michigan Department of Community Health (MDCH) Program will implement the Capacity Building Grant Program to improve awareness about health disparities and social determinants of health, strengthen the leadership and capacity of participating organizations, and encourage partnerships between organizations. To this end, MDCH will hold annual meetings with Capacity Building Grant Program grantees, provide technical assistance and monitor Grant Program initiatives. Also, the project will conduct semiannual grantee trainings. The intended outcomes of this project include increasing participant awareness and skills, stronger program and organizational leadership and increasing capacity for government and communities to address health inequities. In order to measure if these outcomes have been reached, MDCH will conduct pre/post surveys and collect data from such sources as grantee program summaries and activity logs, and participant self-assessments.
