This State Partnership project seeks to implement an evidence-based, systems-level approach to address social determinants of health contributing to health disparities in minority populations, especially within the African-American community. The project will focus on addressing diabetes and infant mortality by encouraging community partnerships and increasing knowledge and awareness about health disparities and quality health outcomes. Through evaluation, the project will collect data on such performance measures as the number of interventions addressing knowledge and awareness, the number of individuals who participate in group education, and the number of persons with increased participation in minority health activities.
