The National Network of Public Health Institutes (NNPHI) National Umbrella Cooperative Agreement (NUCA) Project will address the lack of resources and data for identifying health disparities in minority populations by updating, enhancing and disseminating the Tool for Health and Resilience in Vulnerable Environments (THRIVE) tool to NNPHI member organizations and affiliates who serve minority individuals and communities. This tool was developed to help people understand and prioritize the factors that can help improve health and safety within their own communities. NNPHI will partner with the Prevention Institute to update the THRIVE Toolkit, train THRIVE trainers of trainers (TOT), implement the THRIVE toolkit in 16 TOT-led sites, and disseminate the THRIVE initiative to address health disparities in minority communities.
The intended outcomes include an updated THRIVE tool, which will help mitigate adverse social determinants of health within minority communities across the United States. In order to determine if such outcomes are reached, data will be collected using surveys and interviews to measure programmatic impact. Work plans, agendas, programs and administrative information will also be reviewed to measure program progress and efficacy.
