Brooklyn Partnership to Drive Down Diabetes (BP3D)

The BP3D project will address disparities in obesity-related and diabetes-related health outcomes among racial and ethnic minorities in African American and Latino communities in the low-income, urban neighborhoods of Central Brooklyn and East New York City. The project seeks to establish a community-based, multidisciplinary network that employs evidence-based intervention strategies, addresses social determinants of health and increases access to and utilization of preventive health care, medical treatment and supportive services. To this end, CAMBA will partner with the Greater Brooklyn Health Coalition, Brooklyn District Public Health Office and Community Healthcare Network to provide case management, nutrition and physical fitness services to the target population. Data will be collected through surveys and pre/post tests on a weekly basis.
