State Partnership Grant Program to Improve Minority Health The purpose of the State Partnership Grant Program to Improve Minority Health is to demonstrate the effectiveness of strategic partnerships to improve the status of minority populations and eliminate disparities in at least one of the following health topics: access to healthcare, asthma, cancer, cardiovascular disease/stroke, immunizations, diabetes, HIV/AIDS, infant mortality/LBW, mental health and/or obesity.
