The Health Promotion Council (HPC) intends to develop and implement the Naveguemos con Salud (NCS) Breast Health Partnership Project, a coordinated education and care program, to improve breast health outcomes among Philadelphia's Latina population. The project aims to address the factors that contribute to high rates of breast cancer among Latina women who are less likely to be diagnosed at an early stage and more likely to die of breast cancer than non-Hispanic women. To this end, HPC is partnering with The Family Planning Council and Public Health Management Corporation to provide education and outreach, and create a community-based volunteer network to assist and support women with breast cancer as they move through the healthcare system. The intended outcomes of this program include increasing access to and utilization of medical treatment, and increasing awareness of breast health, breast cancer and early detection among the target population. In order to determine if such outcomes are reached, data will be collected on a range of performance measures, including the number of individuals receiving breast health services. Qualitative and quantitative data will be collected using patient navigator records and patient feedback surveys, as well as an annual partner survey.
