Project Period: 3 years, September 1, 2010 – August 31, 2013
Purpose: : To address social determinants and environmental barriers to healthcare access; and increase access to and utilization of preventive health care, medical treatment, and supportive services through integrated community-based networks. The program will address at least one (but no more than three) of the following health areas: asthma, behavioral health, cancer, cardiovascular disease, diabetes, HIV/AIDS, infant mortality, obesity, substance abuse, and unintentional injuries/violence.
Eligibility: The applicant must be a private nonprofit, community-based, minority-serving organization with a minimum of 5 years experience providing health, human, or social services. The applicant will serve as the lead agency for the project, and will represent a Community-Based Network of at least partners including
- A community-based minority serving organization,
- A local social service organization, and
- A Federally Qualified Health Center.
