Description of Organization:
Life Foundation is a non-profit organization dedicated to stopping the spread of HIV and to assisting people who are living with HIV and AIDS. Its mission is to stop the spread of HIV/AIDS, to empower those affected by HIV/AIDS to maximize their quality of life, to provide leadership and advocacy in responding to the AIDS epidemic and to apply the skills and lessons learned from the AIDS epidemic to other related areas of public health and concern. Since opening its doors as Hawaii's first AIDS organization in 1983, Life Foundation has been implementing innovative programs such as AIDS case management, school-based AIDS education, sterile needle exchange, peer support for positive people, treatment education and peer-to-peer HIV prevention outreach.

Description of project:
The overall proposed program objectives seek to improve the health status of Native Hawaiians, Asians and Pacific Islanders through targeted HIV prevention (risk reduction, counseling and testing) and care services (link HIV positive individuals to medical care, traditional healing practices and other social services as needed). The program will utilize a community partnership model with unique attributes to achieve the proposed objectives. Proposed program objectives include increasing knowledge of HIV transmission and serostatus, reducing risk of HIV transmission, increasing access to traditional native healing services and medical care access as outlined in the application. Life Foundation partners with Waikiki Health Center and Waianae Coast Comprehensive Health Center.
